Job Description

This position may include but is not limited to coverage of areas in Burnsville, Apple Valley and South St. Paul.
FLSA Status:
Hourly, Non- Exempt FTE:
.5 PT 20 hours/week Flexible Schedule to meet the needs of the sites assigned; may include weekends as needed
Hiring Range:
Starts at $40/hour depending on qualifications, experience in the field, skills and education acquired

Thomas Allen (thomasalleninc.com) Perform medical patient assessments and assist in developing individualized care plans Advise and administer medications, treatments, and therapies, as prescribed Monitor patient progress and provide ongoing evaluations and updates to the care team Educate patients and unlicensed direct care employees on health management and home care procedures Coordinate care with other healthcare professionals and community resources Maintain accurate and timely documentation of patient care and progress Perform other duties as assigned, contributing to the success of the program and the overall mission of the organization Valid RN license in Minnesota 2 years of nursing experience or equivalent, preferably in home health or community health settings. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills. Ability to work independently and manage time effectively.
