Job Description

Home Health RN Case Manager•Full Time•Jefferson County Area Alternative Home Health is seeking a passionate RN to join our growing team. As a RN, you would be able to work independently, be dependable, compassionate and have a desire to help others. The Registered Nurse (RN) will travel to patients' homes and be responsible for administering individualized patient care.

RN CASE MANAGER

RESEPONSIBILITIES:
Provide skilled services/education for the patient to achieve their optimal level of health while remaining in their home. Work with a team and implement a care plan for each patient. Communicate effectively with patients, families, physicians, and other healthcare providers. Assess patient's physical and mental status; initiate preventative and rehabilitative nursing procedures as appropriate. Observes signs and symptoms and reports to the physician reactions to treatments, including drugs and changes in the patient's physical or emotional condition. Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home. Maintains accurate and complete records of observations, treatments and care of patient. Takes on-call duty rotation; ranges from every 3rd to 4th weekend.
