Job Description
HARRISON COUNTY HEALTH DEPARTMENT AND HOSPICE IS SEEKING A FULL TIME HOSPICE & HOME HEALTH REGISTERED NURSE
Job Summary:
The Hospice and Home Health Registered Nurse will be a professional in charge of providing care, comfort, and support to clients navigating the difficult time at the end of their lives. The Registered Nurse will develop plans and individual patients to ensure their needs are met, perform regular checkups and communicate with family members as needed.Qualifications:
- Current Missouri Registered Nurse License,
- Current CPR certification,
Brief Summary of Duties:
( A full list of duties may be requested.)- Working knowledge of the principles and practices of professional nursing.
- Working knowledge of inter-professional relationships in the implementation of programs.
- Participates in the assessment of client and family needs; assumes responsibility for managing the care and ensuring needs of patients are met at all times;
- Makes the initial evaluation visit and regularly reevaluates the patient's nursing needs; completes necessary assessments (OASIS for home health);
- Initiates the plan of care and necessary revisions; provides timely updates to primary physician/medical director, other staff and families as needs arise and conditions change;
- Initiates appropriate preventive and rehabilitative nursing procedures;
- Coordinates services; provides those services requiring substantial specialized nursing skills.
- Counsels the patient and family/significant others in meeting nursing and related needs.
- Refers to other therapies or MSW for those patients requiring specialized skills.
- Training and supervision of ancillary nursing personnel as well as supervision of LPN as required by state law; develops hospice CNA plan of care and assignment; along with supervision every 14 days;
- Ability to establish and maintain effective working relationships with clients, professional staff, other employees, and the general public.
- Ability to maintain records, accurate charting, and prepare reports as required and within the appropriate timeframe; Processes orders and notifies physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries
- Appropriately understands and utilizes ICD-10 codes.
- Takes on call duties as assigned to ensure 24 coverage of all patients.
- Perform other related work as assigned.
- Attends IDG meetings as required;
- Knowledge of the county, state, and/or federal regulations that apply to assigned programs (home health and hospice CoP).
- Understands and promotes principles of QAPI.
- Attends/completes required staff trainings, education programs, and in-services to keep up to date on policy and industry changes; presents in-services as requested
