Citizens Memorial Hospital (CMH) is recruiting a Registered Nurse (RN), Licensed Practical Nurse (LPN) or Medical Assistant (MA) to care for our patients in the Pleasant Hope Family Medical Center. CMH is made up of 34 primary and specialty clinics, 6 Long-Term Care facilities and 1 residential care facility. Department:

This rural health clinic (RHC) provides complete family care, well baby exams, well woman exams, Medicare wellness visits, behavioral health visits, immunizations and same day appointments. This clinic is recognized by The Compliance Team as an Exemplary Provider®. A typical day will include:

The RN, LPN or MA is an integral piece of the healthcare team, working with, the physician, and the nurse practitioner.

The role is responsible for:

intake of patient reason for visit, medication reconciliation, verification of the patient's medical/family/social/surgical history, assessment of patient utilizing required questionnaires, assisting providers with in-office procedures, providing education for patient and/or caregiver, providing phone triage, and promoting preventative measures to optimize the patient outcome.
