General Description of Duties and Responsibilities:
The Community Registered Nurse works as a member of the Administrative Support Team, providing medical oversight of clients through direct observation and consultation. Assists Executive, Director of Operations, Program Director, and Quality Assurance Specialist with medical oversight of Team Coaches and Life Coaches in regard to medication administration, storage, re-order, disposal, medication errors, medical documentation, and other duties as necessary.
Daily Duties:
1. Provide appropriate delegation and supervision of Unlicensed Assistive Personnel (UAP) who perform such duties as medication administration and other nursing tasks when applicable, and document those activities. (Nursing Practices Act Chapter 335 RSMo). 2. Ensure L1MA certified staff are competent to administer medications, periodically inspect the technique and skill of a sampling of certified aides delegated to pass medications, and determine a plan of action, in conjunction with medical supervisors and administrator, when certified staff have excessive medication errors. 3. Monitor the application of physician orders including but not limited to treatments, procedures, labs, appointments, specialized diets, medications, outcomes, and adaptive equipment for each resident. 4. Consult on difficult or complex care cases, keeping habilitation/support teams informed of individual health and safety needs. Report any identified deficiencies in the standard of care provided to consumers to Executive Director. Communicate any concerns to physicians as needed. 5. Maintain communication with agency management and habilitation teams, reporting and documenting changes in the consumer's health status, needs and identified deficiencies in the standard of care provided to residents. 6. Assess each resident to evaluate health concerns and identify changes in their health status. Assessment will include face to face visits, interviews with support staff, and review of all documentation. The RN will determine the type of data needed to evaluate health care needs and determine depth of physical assessment required based on known concerns and risks. 7. Monitor the health and safety of each individual client. 8. Implement and maintain statewide health and safety initiatives.
Weekly Duties:
1. Work closely with QA specialist to ensure Medical records are complete and accurate. 2. Assist QA specialist with filing medical paperwork into Medical records (electronic and paper).
Monthly Duties:
1. Complete monthly RN review for each individual and send to Prog Director by 10th of each month. 2. Review/update physician orders, medication administration record (MAR), Health Tracking, any health-related objectives for each individual. 3. Inspect medication administration application of procedures including medication storage. 4. Provide a minimum of 1.25 hours per month per client of nursing oversight. 5. Maintain documentation of monthly expended time and activity for auditing purposes using documentation forms provided by the Department of Mental Health. 6. Review medical inventory 7. Ensure each home has an accurate MAR configured for each individual by the first of each month to ensure appropriate documentation. There should be a blank MAR printed for each individual in the home (as back up in case power or Therap goes down for a time). 8. Must conduct at least 2 initial L1MA trainings and 1 refresher L1MA training per month to potential/new and existing employees of Vantage Supported Living. As needed
Job Duties:
1. Obtain an initial health history and perform initial physical examination within four (2) weeks of admission to a Vantage Supported Living ISL. 2. Attend training provided by the Department of Mental Health, DDD on statewide administration of best practices and expectations of the quality health care system, if necessary. 3. Provide education regarding health matters to Executive Director and other Vantage Supported Living staff as needed. 4. Meet regularly with the Life Coaches to coordinate and direct health care of all ISL clients. 5. Other duties requested by the Executive Director or mandated to the agency by the Department of Mental Health/Division and Developmental Disabilities. 6. Work with staff to ensure requirements for visits are met, review documentation, ensure any and all nursing delegations are completed in a timely manner, all requests made in monthly reviews are met in the following month 7. Ensure mandatory appointments are scheduled within timeline needed, ensure coverage delegating to staff as needed and ensuring completion. 8. Maintain accurate accounts of medical appointments, order changes, summary of appointments, and educate staff to information 9. Notify family/guardian of medical changes if necessary 10. All Other Duties as Assigned Physical Expectations 1. This position requires extended periods of sitting with some periods of walking and standing. It may also require bending, kneeling, and/or stooping and climbing stairs. An individual in this position will need to drive a vehicle to resources in the community. There may be extended periods of using a computer and/or calculator. Working Conditions 1. Office/home environment with noises from those we support, office equipment, and telephones. May be exposed to bloodborne pathogens and/or infectious diseases.
Qualifications:
1. Must maintain an active license in good standing issued by the Missouri State Board of Nursing. 2. Must have knowledge of the unique physical needs and medical implications of individuals with developmental disabilities. 3. Ability to pass a family safety care registry screening. 4. Ability to show strong work ethic, accept supervision and constructive feedback, as well as provide constructive supervision and feedback. 5. Reliability, punctuality, and flexibility in scheduling. The needs of the individuals being served must be met at all times. 6. Interpersonal skills to engage in appropriate interaction with individuals served, team, and people in the community and/or other agencies and entities. 7. Ability to lift over 50 lbs., bend, sit, stand, and take stairs if necessary. 8. Valid Class E driving license, full coverage insurance. 9. Ability to complete all agency required training and certifications within 90 days of hire (CPR/First Aid certification, Level One Med Aid Trainer Certification, Abuse/Neglect training, etc.) as required by DMH.
