Job Description

Three Rivers Hospice in Cassville is actively seeking a competent Registered Nurse with 1-2 years nursing experience to case manage hospice patients. In addition to the Cassville area, you will be visiting Golden, Shell Knob and Branson areas. As an RN Case Manager, you'll be the steady, compassionate presence that patients and families rely on. You'll coordinate care plans, manage symptoms, and ensure that each patient's final chapter is lived with comfort, respect, and peace. Your role will be central to creating moments of connection, relief, and dignity - both for those receiving care and for their loved ones.
Key Responsibilities:
Continuously evaluates personal and professional performance and makes necessary changes to increase productivity and quality of care delivered. Adheres to and supports the policies/procedures/goals/objectives of hospice in an attempt to assure quality patient care. Develops the patients Plan of care in collaboration with other professionals and implements treatment strategies that promotes physical, psychosocial, emotional, and spiritual well-being. Maintains patient confidentiality at all times. Be familiar with the concepts and needs of patients and families who are facing death and dying issues. Effective communication skills, both verbally and written. Identifies the patient's and family's physical, psychosocial and environmental needs and re-assesses as needed Initiates and coordinates the plan of care. Documents problems, appropriate goals, interventions and patient and family response to hospice care. Collaborates with the patient and family, attending physician and other members of the IDG in providing patient and family care on a daily basis. Instructs and supervises the patient and family in self-care techniques when appropriate. We believe every person deserves dignity, comfort, and compassion at the end of life.
