LPN / RN
Nurse for Hospice / Home Health Care Opportunity Alternative Hospice is seeking a passionate LPN/RN to join our growing team. As an LPN or RN , you would be able to work independently, be dependable, compassionate and have a desire to help others. The Licensed Practical Nurse ( LPN ) or Registered Nurse ( RN ) will travel to patients' homes and be responsible for administering individualized patient care and perform duties related to hospice and palliative care in accordance with physician orders. you are under the direction and supervision of a Registered Nurse.
LPN / RN
Nurse Responsibilities Demonstrate knowledge of personal hygiene techniques Work with a team and implement nursing care plan for each patient. Communicate effectively with patients, families, physicians, and other healthcare providers.
LPN / RN
provides physical care to patients in a home- or community-based setting. Assess patient's physical and mental status; initiate preventative and rehabilitative nursing procuedures as appropriate. Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional conidition. Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
LPN / RN
maintains accurate and complete recrods of observtions, treatments and care of patient. Takes on-call duty rotation; nights, weekends, holidays rotated out between all nurses. Attends staff meetings, patient care conferences and inservices as scheduled.
