Job Description

Overview Join our dynamic healthcare team as a Home Health Re-assessor, where you will play a vital role in evaluating and updating patient care plans to ensure optimal health outcomes. This position offers an exciting opportunity to utilize your extensive clinical expertise in diverse settings, including inpatient, outpatient, and post-acute care environments. As a key member of our team, you will conduct comprehensive patient assessments, review medical records, and collaborate with multidisciplinary teams to facilitate seamless care plan approval and utilization management. Your experience and knowledge from an LPN, RN, or bachelor's degree will directly impact patient journeys and quality of life. Have full control of your schedule and join our team! Duties Perform detailed re-assessments of patients' health status through thorough evaluations of physiology and medical history. Completing required home visits scheduled by you and the client weekly One 4-hour office day per week of your choosing Collaborate with healthcare providers to develop or update individualized care plans aligned with current clinical findings. Facilitate discharge planning by coordinating with case management teams to ensure smooth transitions from hospital to home or other care settings. Utilize your education experience to evaluate complex cases involving home health in geriatrics and pediactrics. Maintain compliance with HIPAA regulations and ensure accurate documentation within medical records systems. Engage in utilization management activities by assessing patient needs for appropriate resource allocation and care delivery. Qualifications Registered LPN, Registered Nurse, or Bachelor's Degree Must be able to pass Reassessor state test within the first week of employment Strong background in home health, skilled nursing, or hospital units. Demonstrated expertise in patient assessment, vital signs monitoring, and physiology knowledge. Experience with discharge planning, case management, and diagnostic evaluation processes. Familiarity with medical terminology, HIPAA regulations, and medical records documentation standards. Knowledge of outpatient clinics and post-acute care services including home health or outpatient therapy programs. Excellent communication skills for collaborating with multidisciplinary teams and engaging patients effectively. Ability to handle complex cases involving geriatrics or behavioral health issues.
