Job Description

Three Rivers Hospice in Jefferson City is seeking a compassionate and highly skilled Hospice Registered Nurse (RN) Case Manager to join our dedicated team. In this vital role, you will provide comprehensive nursing care and case management services to patients receiving hospice care, ensuring comfort, dignity, and quality of life during their end-of-life journey. You will coordinate care plans, assess patient needs, and collaborate with interdisciplinary teams to deliver personalized support that aligns with patients' wishes and medical requirements. Your expertise in acute and post-acute care settings will drive exceptional patient outcomes and foster meaningful connections with families and caregivers. This position serves patients throughout Columbia, Jefferson City, Lake of the Ozarks, Owensville Regions , providing meaningful, one-on-one care to individuals and families during one of life's most important journeys. As an RN Case Manager, you will be the steady, trusted presence patients and families rely on—coordinating care, managing symptoms, and ensuring each patient experiences comfort, dignity, and peace. What You'll Do Provide comprehensive nursing care to hospice patients in home and facility settings Develop, implement, and manage individualized plans of care in collaboration with the interdisciplinary team Perform ongoing assessments and adjust care based on patient and family needs Manage symptoms and provide education, guidance, and emotional support Coordinate care with physicians, team members, and caregivers Document patient status, interventions, and outcomes accurately and timely Educate and support families on care techniques and end-of-life expectations Key Responsibilities Continuously evaluate personal and professional performance to improve care quality and productivity Follow and support all hospice policies, procedures, and standards of care Maintain strict patient confidentiality at all times Identify and respond to patients' physical, psychosocial, emotional, and environmental needs Collaborate daily with patients, families, physicians, and the interdisciplinary group (IDG) Provide compassionate care aligned with the unique needs of patients facing end-of-life What We're Looking For Active Missouri RN license Minimum of 1-2 years of nursing experience (hospice or home health preferred) Strong clinical judgment and critical thinking skills Excellent communication and documentation abilities Compassionate, patient-centered approach to care Reliable transportation, valid driver's license, and current auto insurance Why Join Three Rivers Hospice?
