Job Summary:
We are seeking a talented and experienced Registered Nurse to join our assessment team as a Minimum Data Set (MDS) Coordinator. As a key member of our care team, you will be responsible for conducting admission/skilled assessments to develop individualized care plans, ensuring our residents receive the highest level of personalized attention and care.Responsibilities:
Conduct thorough assessments of residents, including MDS assessments, to identify care needs and develop individualized care plans Collaborate with interdisciplinary team members to ensure effective care delivery and coordination Monitor and evaluate resident outcomes, adjusting care plans as necessary to ensure optimal health and well-being Maintain accurate and timely records of resident assessments, care plans, and outcomes Participate in quality improvement initiatives to enhance the overall quality of care and services Will be part of the on-call rotation with weekends as neededRequirements:
Current Registered Nurse (RN) licensure in the State of Missouri, or current multistate license Experience as a MDS RN, with a minimum of 2 years in a similar role Strong assessment and documentation skills Effective communication and interpersonal skills Ability to work collaboratively as part of a multidisciplinary team Strong problem-solving and critical thinking skillsWhat We Offer:
