Job Description

Join our compassionate healthcare team as an RN Hospice Nurse Case Manager, where you will play a vital role in delivering personalized, holistic care to patients facing life-limiting illnesses. This dynamic position combines clinical expertise with case management skills to ensure seamless coordination of services, promote comfort, and enhance quality of life for patients and their families. As a key member of our interdisciplinary team, you will advocate for patient needs, facilitate effective communication, and oversee comprehensive care plans in both inpatient and outpatient settings. Your dedication will make a meaningful difference during some of the most vulnerable moments in patients' lives. Conduct thorough patient assessments, including vital signs, physiological evaluations, and medical history reviews to determine care needs. Develop and implement individualized care plans aligned with hospice standards, ensuring compliance with HIPAA regulations and medical terminology accuracy. Coordinate discharge planning, post-acute care, and outpatient services to facilitate smooth transitions across care settings. Collaborate with interdisciplinary teams—such as physicians, social workers, and therapists—to optimize patient comfort and address complex medical issues like infectious diseases or behavioral health concerns. Manage intake processes for new hospice patients, including diagnostic evaluations and utilization management to prioritize urgent needs. Monitor patient progress through ongoing evaluation of symptoms, medication management, and response to interventions; adjust care plans accordingly. Maintain detailed medical records and documentation of patient interactions while adhering to HIPAA privacy standards. Provide education to patients and families about disease processes, symptom management, and end-of-life planning to support informed decision-making. Valid RN license with current registration; additional certifications in hospice or palliative care preferred. Critical care experience such as ICU, emergency medicine, or Level I/II trauma center background highly desirable. Experience in hospital medicine, medical-surgical units, or post-acute care environments including inpatient or outpatient settings. Knowledge of physiology, diagnostic evaluation techniques, and case management principles essential for effective patient assessment. Familiarity with managed care processes, utilization management, discharge planning, and medical records documentation. Strong understanding of HIPAA regulations and medical terminology related to hospice and palliative services. Demonstrated ability to perform patient service tasks such as vital signs monitoring and comprehensive intake assessments. Excellent communication skills with the ability to coordinate multidisciplinary teams across hospital experience or clinic settings. Geriatrics, pediatrics (PICU), behavioral health or infectious disease care experience is a plus for addressing diverse patient populations. $35.00 - $40.00 per hour
