MDS Coordinator- RN required

We're seeking a dynamic and detail-oriented MDS Coordinator (RN) to join our RSP Senior Living Team for our Rolla Skilled Nursing Facility.

MDS Qualifications:

Current and valid Missouri RN license Minimum 1 year of prior MDS Coordinator experience Experience in Long-Term Care, SNF or comparable a plus Self-starter with strong time management skills Excellent communication and relationship-building abilities

MDS Essential Duties:

Complete and monitor MDS assessments in accordance with state and federal guidelines Collect and organize patient data for reporting and care planning Schedule and facilitate care plan conferences Guide completion of

PPS, OBRA

assessments, Triggers, and RAPs Submit MDS assessments timely and maintain documentation for surveys Serve in a nursing capacity as needed Develop and oversee a resident-centered quality assurance program Observe resident care, communicate with nursing staff, and ensure documentation and care plans remain up to date.
