Job Description

Job Summary We are seeking a dedicated and energetic 3-11/11-7 FULL
TIME OR PART TIME
Nurse (RN/LPN) to join our dynamic healthcare team. In this vital role, you will provide comprehensive patient care during evening and overnight shifts, ensuring the safety, comfort, and well-being of diverse patient populations. Your expertise will support acute, chronic, and long-term care needs across various hospital, clinic, and community-based environments. Responsibilities Deliver high-quality nursing care by performing patient assessments, vital sign monitoring, and physical examinations with precision and compassion. Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards. Utilize EMR (Electronic Medical Records) systems such as PCC to accurately document patient information, medical histories, and treatment plans. Manage cases involving dementia care, Alzheimer's care, memory care, hospice care, and acute pain management with sensitivity and professionalism. Assisting in diagnostic evaluations including specimen collection, catheterization, and scheduling lab and x-rays. Collaborate with interdisciplinary teams for discharge planning, case management, and coordination of continuous resident care after discharge. Maintain compliance with HIPAA regulations and infection control standards to ensure patient confidentiality and safety at all times. Qualifications Valid nursing license (RN or LPN) with current certification to practice in your state. Preferred but not required-TRAINING
PROVIDED FOR AREAS NOT FLUENT
Strong background working with diverse populations including pediatrics, geriatrics, individuals with disabilities or developmental disabilities. Familiarity with EMR/EHR systems like PCC for documentation purpose. Experience in
LTC/GERIATRICS

Skills in medication administration including respiratory treatments,injections and non complex wound care Excellent patient assessment skills combined with physiology knowledge and understanding of anatomy for effective diagnosis and treatment planning.
