Duration:

13 Weeks Guaranteed hours: 40 Responsibilities Perform thorough patient assessments, including vital signs, physical examinations, and review of medical records to determine care needs. Develop and implement individualized care plans in collaboration with physicians, case managers, and other healthcare professionals. Manage acute and chronic conditions across diverse patient populations such as geriatrics, pediatrics, behavioral health, and post-acute care. Conduct medication administration, wound care, IV therapy, and other specialized nursing procedures following best practices and safety protocols. Coordinate discharge planning and facilitate smooth transitions from hospital or inpatient settings to home-based care. Monitor patient progress through diagnostic evaluations and adjust care plans accordingly. Educate patients and their families on disease management, medication adherence, and health maintenance strategies. Ensure compliance with HIPAA regulations and maintain accurate medical records in accordance with regulatory standards. Participate in utilization management activities including intake assessments and case management to optimize resource use. Collaborate with hospital teams for inpatient or outpatient services as needed to ensure continuity of care.

Requirements:

Homecare Homebase Software Experience

• Required OASIS Experience
• Required Current state license as a Registered Nurse
• Required CHPN certification
• Preferred Proof of current CPR certification
• Required Current driver's license
• Required Must have reliable transportation and insurance
• Required 1-2 years experience as a Registered Nurse in a clinical care setting
• Required 2+ years Home health experience
• Required.
