Case Manager Locations:
Jackson, MS Duration:
13 Weeks with extension
Shifts:
Mon to Fri 8:00 AM•4:30
PM Pay:
$2600/week local
Pay:
$1840/week Responsibilities Perform discharge planning for medical and some social cases. Coordinate referrals using naviHealth (transitioning to Epic Care Management). Collaborate with physicians, nursing staff, pharmacy, respiratory therapy, and social work teams. Ensure discharge readiness and assist with complex placements (contracted facilities available). Educate patients and families on discharge plans and follow-up care. Requirements Active RN state (MS) / Compact state license Recent 2 years of Case Management experience in Acute Facility (Hopsital or Medical Center) EMR system :
EPIC BLS
certification Trauma Level I or II experience
