Position Overview As a Home Health Case Manager, you will play a key role in collaborating with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload. This is a direct care role, involving travel within Boise, ID and surrounding regions to deliver in-home care from admission to discharge. Your work will ensure that each patient receives the highest standard of care, aligned with their needs and the care plan. This Role Full-time Schedule :
Monday to Friday, 8:30 AM - 5:00 PM, with rotating evening on-call and weekend shifts
Patient Population :
Adults - interest in training for an occasional pediatric patient
Manageable Caseloads :
Daily visit requirements are below the state and national average
Compensation :
Paid hourly which includes drive time and mileage reimbursement for traveling within Boise and it's surrounding regions
Additional Pay :
On-call pay and applicable differential pay
Bonuses :
Sign-on bonus and relocation assistance available (where applicable)

Qualifications Education :
ASN or BSN in
Nursing Experience :
2 years of RN experience, Hospice Case Managerment experience preferred
Licenses/Certifications :
Current RN license in the state of Idaho; Basic Life Support (BLS) certification
Other Requirements :
