Qualifications/Desired Experience:
Current, unencumbered RN license Minimum of one year experience in rehab or long-term care nursing preferred Proficiency in comprehensive nursing assessment, care planning, and interdisciplinary communication Demonstrated commitment to patient and employee safety Strong documentation and time management abilities
Key Responsibilities:
Complete thorough nursing assessments within 8 hours of admission Develop, implement, and regularly update individualized care plans Collaborate with patients, families, and interdisciplinary teams to identify needs and goals Communicate changes in patient status and share information with other care providers to ensure continuity and safety Participate directly in patient rehabilitation and implement evidence-based interventions Initiate discharge planning and patient education from the outset of admission Supervise and delegate tasks effectively to ancillary nursing personnel Contribute to a positive, efficient unit by fostering teamwork and reassurance Maintain high standards of documentation, appearance, attendance, and punctuality Embrace professional development through ongoing education and training
