Job Description

Overview We are seeking a dedicated and compassionate Long Term Care (LTC) Registered Nurse to join our healthcare team. In this vital role, you will provide comprehensive nursing care to residents in long-term care settings, including nursing homes, assisted living facilities, and memory care units. Your expertise will support the health and well-being of individuals with diverse medical needs, including those requiring specialized treatments such as IV infusion, wound care, and medication management.

Stipends:
$1246/week W2: $1170/week Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and health histories to develop individualized care plans. Administer medications and treatments accurately, including injections, IV infusions, wound suturing, and catheter care. Monitor patients for signs of acute pain, infectious diseases, or changes in condition; respond promptly to emergencies. Collaborate with interdisciplinary teams to coordinate care plans, discharge planning, and case management. Maintain detailed medical documentation in compliance with HIPAA regulations and facility policies using electronic health record (EHR) systems such as Cerner or Athenahealth. Provide specialized care for residents with memory impairments or behavioral health needs; support dementia and Alzheimer's care initiatives. Educate patients and their families on health management strategies, medication adherence, and post-discharge instructions. Ensure aseptic techniques are followed during procedures such as phlebotomy, specimen collection, and sterile dressing changes. Participate in quality improvement activities aligned with NCQA standards and ICD coding practices. Support infection control protocols and maintain a safe environment for residents and staff. Qualifications Valid Registered Nurse (RN) license in good standing with relevant state licensing boards. Minimum of two years of nursing experience in long-term care facilities or hospital settings; experience with hospital medicine or critical care preferred. Proficiency with EMR/EHR systems such as Cerner or Athenahealth; familiarity with
ICD-10/ICD-9
coding is advantageous. Demonstrated expertise in medical-surgical nursing, geriatrics, hospice & palliative medicine, or behavioral health. Strong clinical skills including IV insertion, medication administration, wound suturing, tube feeding management, and ventilator support. Knowledge of trauma medicine levels I & II trauma centers experience is a plus. Ability to work effectively with individuals with disabilities or developmental challenges across diverse populations. Excellent communication skills for patient education and interdisciplinary collaboration. Commitment to maintaining HIPAA compliance and practicing infection control measures diligently. Additional certifications such as Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), or Critical Care Transport certification are desirable.
