Job Description

Overview Join our dynamic healthcare team as a Long Term Care (LTC) Swing Registered Nurse, where your expertise will make a meaningful difference in the lives of residents across diverse care settings. This role offers the opportunity to work flexibly across multiple facilities, providing high-quality nursing care in long-term care, assisted living, memory care, and post-acute environments. As a vital member of our team, you will deliver comprehensive patient assessments, administer medications and treatments, and coordinate care plans to promote optimal health outcomes. Your energy and dedication will help foster a compassionate, safe, and engaging environment for residents with complex needs. Responsibilities Perform thorough patient assessments, including vital signs, physical examinations, and physiological evaluations to determine individual care needs. Administer medications and treatments accurately, including injections, IV infusions, tube feedings, and wound suturing while adhering to aseptic techniques. Manage acute pain and provide specialized care for residents with dementia, Alzheimer's disease, or behavioral health conditions. Collaborate with interdisciplinary teams to develop, implement, and update personalized care plans aligned with residents' goals. Utilize electronic health record (EHR) systems such as Epic or Cerner to document medical records meticulously and ensure compliance with HIPAA regulations. Conduct diagnostic evaluations including laboratory specimen collection/processing and assist with procedures like catheterization or ostomy care. Support residents' mobility and safety by operating equipment such as Hoyer lifts and ventilators while maintaining proper safety protocols. Engage in triage activities for urgent health concerns, prioritizing resident needs based on severity and urgency in fast-paced environments like emergency rooms or urgent care settings. Provide education on health coaching topics such as nutrition, medication management, or chronic disease control to residents and their families. Qualifications Valid RN license with current registration; experience working in long-term care facilities is highly preferred. Demonstrated proficiency with EMR/EHR systems including Epic or Cerner; familiarity with
ICD-9/10
coding is advantageous. Critical care experience in ICU or emergency medicine settings is desirable; experience with trauma medicine or Level I/II trauma centers is a plus. Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques. Skilled in performing complex procedures such as sterile processing, phlebotomy, IV insertion/infusion, suturing, and airway management. Experience working with individuals with disabilities or developmental delays; familiarity with memory care or dementia-specific protocols is beneficial. Ability to handle multiple priorities efficiently while maintaining attention to detail; excellent communication skills are essential. Certifications including Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), and relevant specialty certifications are preferred but not mandatory.
