Job Description

We are seeking a compassionate and skilled Hospice Nurse to join our dedicated team. The ideal candidate will provide high-quality nursing care to patients in hospice settings, ensuring comfort and dignity during their end-of-life journey. This role requires a strong understanding of hospice and palliative medicine, as well as the ability to work collaboratively with families and interdisciplinary teams. This position does require daily travel to the surrounding areas that we service. Duties Deliver comprehensive nursing care to hospice patients, focusing on pain management and symptom relief. Conduct thorough patient assessments to develop individualized care plans. Administer medications and treatments as prescribed, including IV infusions and injections. Collaborate with physicians, social workers, and other healthcare professionals to coordinate patient care. Provide emotional support and education to patients and their families regarding end-of-life issues. Maintain accurate medical documentation in compliance with HIPAA regulations. Monitor vital signs and perform necessary medical procedures such as catheter care, phlebotomy, and tube feeding. Participate in case management activities, including discharge planning and coordination of home care services. Ensure adherence to NCQA standards and infection control protocols. Qualifications Current nursing license is required Some experience in Hospice/palliative care is a plus Proficiency in EMR/EHR systems such as Epic or Cerner is highly desirable. Experience in critical care, acute care, or long-term care settings is beneficial. Familiarity with medical terminology, ICD coding (ICD-9 & ICD-10), and CPT coding is advantageous. Knowledge of pain management techniques and acute pain management strategies. Ability to provide compassionate care for individuals with developmental disabilities or dementia. Strong assessment skills with the ability to perform physical examinations and diagnostic evaluations. Experience working in various healthcare environments including hospitals, nursing homes, assisted living facilities, or home care settings is a plus. Excellent communication skills for effective interaction with patients, families, and healthcare teams.
Pay:
$35.00 - $45.00 per hour
