Job Description

Job Summary Join our dynamic healthcare team as a Nurse Navigator, a vital role dedicated to guiding patients seamlessly through their healthcare journey. In this energetic and impactful position, you will serve as a compassionate advocate, coordinating care across various specialties and ensuring patients receive timely, personalized support. Your expertise in acute and chronic care settings will empower you to assess patient needs accurately, facilitate communication among multidisciplinary teams, and promote positive health outcomes. Header Duties Act as the primary point of contact for patients navigating complex healthcare systems, providing education and emotional support throughout their treatment process Coordinate multidisciplinary care plans by collaborating with physicians, specialists, social workers, and other healthcare professionals Conduct comprehensive patient assessments including vital signs, medical history review, and physiological evaluations to determine individual needs Manage patient records with strict adherence to HIPAA regulations, documenting all interactions accurately within electronic medical records systems Assist with diagnostic evaluations and case management activities to optimize utilization of hospital resources and manage care effectively Provide specialized support in areas such as critical care, trauma centers, emergency medicine, infectious disease care, pediatrics, geriatrics, behavioral health, and outpatient clinics Requirements Valid nursing license with current registration in Montana Strong knowledge of hospital medicine, medical-surgical nursing, and acute care procedures Proven experience in patient assessment, discharge planning, utilization management, and case management processes Familiarity with medical terminology, physiology concepts, diagnostic evaluation techniques, and medical records documentation Excellent communication skills with the ability to educate patients and coordinate effectively across multidisciplinary teams Demonstrated understanding of HIPAA compliance and patient privacy regulations Experience working with diverse populations including pediatrics (PICU), geriatrics, behavioral health patients or post-acute care environments is a plus Join us as a Nurse Navigator to be at the forefront of compassionate patient advocacy!
