Job Description

TREK Healthcare We are seeking a compassionate and experienced Part-Time Registered Nurse (RN) Case Manager to join our hospice and home health team. This role is ideal for an individual who values meaningful patient relationships, interdisciplinary collaboration, and providing end-of-life care with dignity, empathy, and excellence. This part-time position is scheduled for 16 hours per week, with the potential to flex up to 30 hours per week, depending on patient needs and census. The standard schedule is Monday and Wednesday or Tuesday and Thursday.
Position Details Schedule:
Tuesday and Thursdays or Monday and Wednesdays One on-call weekend per month Two on-call weekdays per month Key Responsibilities Provide direct nursing care to hospice patients in their place of residence Serve as the primary case manager, overseeing and coordinating all aspects of patient care Manage and collaborate with interdisciplinary team members, including: LPNs, Hospice aides (bath aides), Social workers, chaplains, and other disciplines Develop, implement, review, and update individualized plans of care Complete accurate, timely clinical documentation and charting Follow up on care plans, including reassessments and resubmissions as required Communicate effectively with patients, families, physicians, and other healthcare professionals Provide education and emotional support to patients and families, leading with empathy and compassion Participate in on-call rotation to support patients after hours as needed Ensure compliance with hospice regulations, agency policies, and standards of care Qualifications Current Registered Nurse (RN) MT license Hospice or home health experience preferred (but not required for the right candidate) Strong clinical assessment and care coordination skills Excellent communication, organization, and documentation abilities Ability to work independently while collaborating with a multidisciplinary team Valid driver's license, proof of insurance coverage, and reliable transportation EMR experience preferred Benefits Competitive hourly pay (Pay rate is based on nursing licensure, relevant experience, and qualifications) Paid observed holidays Flexible scheduling Supportive, team-oriented work environment Meaningful work that makes a difference for patients and families Why Join Us?
