REGISTERED NURSE CASE MANAGER
Are you passionate about helping those who struggle to access care or feel unwelcome in other healthcare spaces? Western North Carolina Community Health Services (WNCCHS) is a Federally Qualified Health Center (FQHC) serving underserved communities in Western North Carolina. WNCCHS is seeking a mission-minded Registered Nurse Case Manager to provide care management to patients with Medicare and Medicaid at our Minnie Jones Health Center in Asheville. RN Case Managers collaborate with medical, behavioral health, pharmacy, dental, and care management teams to provide customized care plans to improve health outcomes. The RN Case Manager ensures the successful transition of care from inpatient or the emergency department to culturally competent, comprehensive primary care.
QUALIFICATIONS
NC state RN licensure BLS certification Bachelor's degree in nursing or related field Two years' RN experience required Experience with Medicaid patients Experience in community health or outpatient care preferred Clinical experience in nursing care of the medically complex patient highly desirable Familiarity with role of FQHCs in the community Bilingual in English and Spanish and/or Russian is a plus HIV care experience a plus
COMPENSATION
Base Salary:
$75,000 to $80,000 (based on 40 hours/week)
