Job Description
Nursing degree, active registered nurse (RN) license and minimum 1+ years Home Health RN experience required. Applicants who do not meet these qualifications will not be considered. We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to deliver skilled nursing care to patients in their homes, ensuring high-quality, patient-centered outcomes. In this field-based role, you will leverage your clinical skills, autonomy, and organization to facilitate optimal patient experiences and care plan compliance throughout a 13-week contract.
Schedule:
Monday - Friday, 8:00 AM - 5:00
PM Key Responsibilities:
Conduct comprehensive OASIS assessments and develop individualized care plans. Deliver skilled nursing interventions, including wound care, medication management, IV therapy, and chronic disease monitoring. Monitor patient progress; identify and promptly report any changes in condition to the multidisciplinary team and physicians. Provide education to patients and families regarding diagnosis, medications, and at-home care. Maintain timely, accurate electronic documentation through Home Care Home Base (HCHB). Coordinate and collaborate with therapists, social workers, and home health aides to provide holistic care. Participate in regular case conferences and required continuing education. Ensure compliance with regulatory and agency standards for all clinical documentation and care.
Qualifications:
Active RN license in good standing. At least 1 year of recent home health nursing experience (field-based highly preferred). Proficiency in Home Care Home Base (HCHB) and OASIS required. Robust clinical assessment, critical thinking, and organizational skills. Reliable vehicle, valid driver's license, and current auto insurance. Ability to thrive independently and manage complex schedules. Strong interpersonal and communication abilities.
Preferred:
BSN degree. Experience managing a caseload of 25-32 points per week. Familiarity with Medicare and home health compliance.
