YOUR ROLE
You'll help join a physician led clinical team bringing value based care to complex home bound patients. Our physician practice is focused on helping patients age at home while avoiding the hospital and SNF. Must be comfortable with patient assessments including gathering history, medication reconciliation, symptom and patient request triage. Must be comfortable communicating with provider partners e.g., Home Health clinicians, PCP offices, Subspecialty offices, case managers Must be comfortable leading a team of CMAs Requirements Must have active CHW license At least 3 years of clinical experience Ideal candidate is self starter, detailed oriented and comfortable with modern software This is an in-person role however there is no in-person patient contact
