13 Weeks Job Summary Seeking an experienced RN Care Manager to support discharge planning, utilization review, care coordination, and transition planning for acutely ill patients in a teaching hospital setting. Candidate will collaborate with interdisciplinary teams and assist with complex patient care progression and discharge readiness. Required Skills & Experience Minimum 2 years of healthcare experience as an RN required Previous acute care hospital case management/discharge planning experience preferred Strong assessment and critical thinking skills EPIC experience preferred Experience with interdisciplinary care coordination preferred Required Education Nursing Diploma

from an accredited Nursing program BSN/MSN preferred Certifications & License Requirements Active North Carolina RN License required Additional Information 36 hours/week No weekends, holidays, or on-call required Patient populations include Cardiology, Nephrology, Trauma Surgery, Ortho Surgery, Vascular Surgery, Neurosurgery, GI Surgery, Urology, Transplant, ICU, and Stepdown units CA02
