Job Description

Job Summary RN Care Manager Seeking an experienced RN Care Manager to provide assessment, care planning, utilization review, and discharge planning for patients in an acute care setting. The role involves coordinating with multidisciplinary teams, identifying barriers to discharge, supporting care progression, and ensuring continuity of care to improve patient outcomes and satisfaction.
Requirements:
Active RN license Minimum 2 years of healthcare experience as a Registered Nurse Strong assessment and critical thinking skills
Pay:
$1,900.00 - $2,200.00 per week
