Traveler:

Required:
Minimum 2 years of RN experience — Hospice and/or Home Health setting required Epic EMR proficiency (required) Active BLS — American Heart Association Active North Carolina RN License (or compact license) Flu vaccine required (medical/religious exemptions only)
Duties:
Provide comprehensive outpatient hospice and crisis response nursing care across home health and skilled nursing settings Conduct 5 patient visits per day for patients with terminal diagnoses (primarily geriatric) Perform intake assessments, develop care goals, and implement individualized hospice care plans Manage complex pain and symptom needs using evidence-based pharmacologic and non-pharmacologic approaches Educate and support patients and families through all stages of the end-of-life process Collaborate with interdisciplinary team via Epic Remote Client and SBAR communication framework Participate in on-call rotation: Friday, Saturday, and Sunday 8:00 PM
  • 8:00 AM every 4-5 weeks Respond to after-hours patient PRN and death visit needs as routed through the nurse triage service Mileage reimbursed at IRS standard rate Why Charlotte, NC?
