Skills, Qualifications & Desired Experience:
Current, active RN license (state or compact) Minimum 2 years of recent experience in Case Management Strong assessment, organizational, and communication skills Proficiency with electronic medical records Ability to work independently as well as part of an interdisciplinary team Knowledge of utilization review and discharge planning processesKey Responsibilities:
