Skills, Qualifications & Desired Experience:
Current, active RN license (state or compact) Minimum 2 years of recent experience in Case Management Strong assessment, organizational, and communication skills Proficiency with electronic medical records Ability to work independently as well as part of an interdisciplinary team Knowledge of utilization review and discharge planning processes
Key Responsibilities:
Coordinate comprehensive patient care plans from admission to discharge Collaborate with physicians, social workers, and families to ensure optimal patient outcomes Navigate insurance requirements and authorization procedures Provide patient education regarding diagnoses, treatments, and community resources Support advocacy for patient needs and services
