Job Description

Job Overview We welcome and encourage all to apply, Retired and New Graduates. We are seeking a dedicated and highly skilled Staff Registered Nurse for Home Care Agency. Flexible Choice of two (2) days per week (Monday-Tuesday) or (Tuesday-Wednesday)(8am-1pm) or (9am-2pm). Responsibilities Perform thorough patient assessments, including vital signs, medical history reviews, to prepare individualized care plans. Monitor patient progress during supervisory visits and ensure all emergency information is documented Record patients' conditions continuously, recognizing signs of new pain or deterioration as necessary. Utilize medical terminology accurately. Educate patients and their families on treatment plans, medication administration and their plans of care. Collaborate with multidisciplinary teams—including physicians, therapists, and case managers—to coordinate patient care plans effectively. In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times. Oversee CNAs and companions to ensure compliance, work commitment is performed, CEs are met, monitor patient-CNA work performance Provide comprehensive patient care across diverse healthcare settings including pediatrics and geriatrics—and those with developmental disabilities or dementia. Qualifications/Requirements Proven experience with valid nursing license (RN) with current registration in the relevant jurisdiction. Proficiency have familiarity with
ICD-10/ICD-9
codes. Perform Supervisory Visit monthly, Office and other Regulatory Duties as required Strong knowledge of medical terminology, physiology, and diagnostic evaluation techniques. Strong clinical skills and experience with specialized procedures such as catheterization, ostomy care, oxygen, infection control practices and trach to train CNAs in field. Must complete all State required pre-employment testing Must have good standing NC Nursing License. Must submit to criminal background check, CPR, Basic life support (BLS), valid work document, current TB report or screening, and valid NC driver's license. Must present advanced certifications relevant to specialty areas; ongoing professional development is required. Must maintain current medical knowledge/documentation in compliance with HIPAA regulations Knowledge of occupational health protocols and infectious disease management. Excellent communication skills combined with critical thinking abilities; ability to work effectively within multidisciplinary teams in fast-paced environments. Prior experience in home & community care settings, or assisted living facilities is highly desirable.
