Position Overview The Case Manager II (Registered Nurse)- Pediatrics (Medicaid) is responsible for providing medical case management services to a specified panel of patients within the clinical setting.
T HIS POSITION OFFERS A 40-HOUR WORKWEEK, NO WEEKENDS, FULL BENEFITS PACKAGE, 12 PAID HOLIDAYS, BI-ANNUAL BONUS, BI-WEEKLY PAY.
Essential Tasks Provide medical case management primarily to high-risk children with developmental delays or severe medical conditions (i.e., cancer, asthma, diabetes, cardiovascular disease, etc.) in accordance with CFHC, Inc.'s policies and procedures, Medicaid Managed Care-Advance Medical Home requirements, and clinically integrated network requirements. May also be assigned a small case load of adult patients to case manage. Assists providers in completing age appropriate developmental screening, social determinants of health and other health risk assessments, and provides education and resources directly to patients and/or guardians, if needed. Perform annual comprehensive assessments to evaluate patients' health care needs and social determinants of health to develop individualized and person-centered care plans using a collaborative approach including patient and guardian/family participation, where possible. Collaborate with community agencies for referral resources to assist patients and utilize NC Care 360. Establish and document a care plan with the patient and/or guardian/family members. Update and document care plans as the patients' needs change and/or to address gaps in care. Experience and Education Graduate of an accredited school of nursing as a Registered Nurse. Current and unrestricted license to practice in North Carolina. Previous experience, preferably, in a community health center setting in the area of pediatric nursing and/or case management. Must demonstrate the skills and judgment necessary to provide direct care to patients and nursing interventions under the direct supervision of providers. Strong computer skills and working knowledge of the windows-based environment and Electronic Health Record (EHR) experience. Active North Carolina Driver License. Effective communication and interpersonal skills. Schedule Four days a week (32 hours) 8 AM- 5 PM, 8-hour shift, day shift Four days a week (36 hours) 7:30 AM
