Position Summary Registered Nurse of Care Management is responsible for screening, assessing, planning, monitoring, and evaluating the designated patient's plan of care of the AMH Tier 3 Care Management activities. The RN collaborates readily with the Director of Care Management to monitor, achieve, and surpass the AMH Tier 3 requirements. The RN coordinates services and provides self-management education to patient/caregiver. The RN Care Manager communicates and collaborates with the patient's Primary Care Physician and specialty healthcare providers to provide coordinated Care Management services. The care manager will provide care that is respectful of and responsive to individual patient preferences, needs, and values. Duties and Responsibilities Responsibilities include but are not limited to: Responsible for the patient's plan of care as part of the AMH Tier 3 status including vision and performance of Care Management and outcomes of members Utilizes targeted reports and real-time referrals to identify clients who may benefit from care management services. The frequency and intensity of care management will be determined by the patient's needs with a focus on person centered care that considers the totality of the person being treated and includes, medical, mental, emotional, spiritual, social, cultural, relational, contextual and environmental aspects. Ability to extract data, interrupt information and advise on improvements to ensure metrics for care management are made on a weekly and monthly basis. Must collaborate with the Director of Care Management to access and improve workflows to refine and create more efficient workflows and enhance all patient care activities Provides care coordination, health teaching and promotion, and evaluation of goals which requires on-going communication with the patient, other care team members, the patient's primary care provider, specialty behavioral health providers, specialty care providers, and local community-based organizations and/or others involved in providing care to the identified patient. The care manager will meet performance targets as outlined in policies. Coordinates the effective and efficient delivery of patient care management services through the application of care management principles. Identifies clinical priorities and initiates action to improve patient outcomes. Acts as a patient advocate throughout the continuum of care including all patient settings and in the community. Communicates effectively to promote sharing of information throughout the continuum of care and as an effective member of the healthcare team. Documents assessments, screenings, tasks, interventions, goals, care plans, and medication management in the appropriate database and per department policy and national accreditation guidelines. Maintains strong understanding of chronic disease, behavioral health diagnoses, evidence-based treatment and management, and motivational interviewing techniques and is able to apply these principles in the care management services being provided to patients. Completes mandatory annual trainings and competencies, attends staff meetings as scheduled. Attends continuing education programs related to program goals. Follows hospital and department policies and procedures. Assumes responsibility for keeping informed about changes in policies and procedures Performs miscellaneous job-related duties as assigned. Qualifications Graduate from an accredited School of Nursing (RN). Minimum of 3 year of nursing experience in a clinical setting. Currently licensed by North Carolina as a Registered Nurse Minimum of 3 years in care management experience preferred CCM (Certified Case Manager) preferred Self-motivated, self-directed, and able to work independently with minimal supervision Promotes collaborative work atmosphere Effective leadership, teaching, coaching, and interpersonal skills Effective written and oral communication skills Must have experience with spreadsheet and database program applications Ability to accept and incorporate critical comment Able to work in a continuously changing work environment Attention to
