Northland PACE currently has an opening for a registered nurse to serve in the vital role of a Clinic Nurse Case Manger on our Interdisciplinary Team. As the CNCM , you will coordinate and deliver personalized care for older adults who want to age in place. You'll balance clinical expertise with care coordination, helping our participants maintain independence, dignity and quality of life.

What You'll Do:
Provide nursing case management and care coordination for participants in the PACE clinic and community setting Collaborate closely with the IDT to develop and implement individualized care plans Educate participants and caregivers about medications, chronic disease management, and wellness strategies Support transitions of care from hospital or rehab back to home Advocate for participant needs across care settings
Who We Are Looking For:
A nursing candidate with an RN degree from an accredited college or university and holds an unincumbered nursing license to work in ND. The nursing candidate would have at least two years' experience as a staff nurse preferably with complex and older populations. They would have a passion for serving older adults in a holistic, community-based setting and want to be apart of a caring, mission-driven PACE team! Ready to apply?
