LPN SKILLED NURSING FACILITY
Shift:
- 12 Hours Days 36 hours/week. (3
- 12) 12-week contract Perform thorough patient assessments, including vital signs, physical examinations, and medication reconciliation, ensuring accurate documentation within electronic health records (EHR) systems.
Administer medications and treatments accurately, including injections, IV infusions, catheter care, and wound management, following established protocols and safety standards. Monitor residents' conditions closely, recognizing signs of acute pain, infection, or deterioration, and escalate concerns appropriately through triage procedures. Assist with complex clinical procedures such as tube feeding, sterile processing, and respiratory support including ventilator management. Collaborate with physicians and interdisciplinary teams to develop and implement individualized care plans tailored to residents' needs, including dementia and Alzheimer's care. Maintain compliance with long-term care regulations, HIPAA confidentiality policies, and infection control protocols to ensure a safe environment for residents and staff. Educate residents and their families on health management strategies, medication administration, and discharge planning to promote well-being and autonomy. Requirements Valid licensure as a Licensed Practical Nurse (LPN) in the state of employment with current CPR/BLS certification. Proven experience working in skilled nursing facilities or long-term care environments; experience with geriatrics highly preferred. Familiarity with electronic health record (EHR) systems such as Epic or Cerner is advantageous. Strong knowledge of nursing practices including medication administration, vital signs monitoring, and patient assessment techniques. Experience working with individuals with developmental disabilities or dementia is desirable; knowledge of Alzheimer's and memory care best practices is beneficial. Ability to handle emergency situations effectively while maintaining composure; ICU or emergency room experience is a plus. Excellent communication skills combined with compassionate patient-centered care approach; ability to work collaboratively within multidisciplinary teams.
