Job Description
Essential Duties and Responsibilities Conduct nursing assessments and develop individualized care plans. Provide skilled nursing services in accordance with physician orders and agency policies. Administer medications, treatments, and other nursing interventions as prescribed. Monitor client health status and report changes to physicians, supervisors, and family members as appropriate. Complete accurate and timely documentation of all services provided. Educate clients, families, and caregivers on health conditions, medications, and care plans. Participate in care coordination with interdisciplinary team members. Ensure compliance with state regulations, agency policies, and client rights. Assist with supervisory visits and evaluations of direct care staff when required. Maintain client confidentiality and adhere to HIPAA regulations. Attend required trainings, meetings, and continuing education activities.
