Home Nursing With Heart in Omaha, NE is seeking a skilled RN to join our team as an Admissions Nurse, where you will play a vital role in assessing, admitting, and coordinating care for our patients. If you are passionate about delivering high-quality care with a focus on patient-centered care, we would love to hear from you.
Key Responsibilities:
Help patients with needs over the weekend Conduct in-home assessments to determine eligibility for home health services Collaborate with patients, families, and interdisciplinary teams to create individualized care plans Coordinate with physicians and other healthcare providers to ensure continuity of care Provide patient education and support to promote self-management and independence Ensure compliance with regulatory requirements and quality standards
Qualifications:
