Homecare & Hospice Location:
North Conway, NH Schedule:
Full‑time, Monday-Friday, with rotating on‑call

About the Role As an RN Case Manager , you'll provide compassionate, high‑quality nursing care to patients in the comfort of their homes. You'll guide patients and families through recovery, chronic illness management, or end‑of‑life care, ensuring dignity, comfort, and exceptional clinical outcomes. This is a meaningful, relationship‑driven role where your clinical expertise and emotional intelligence truly matter.
