Provide direct and indirect patient care in accordance with the Nurse Practice Act, professional standards of care, state regulations and accreditation standards and HUHS philosophy, policies, and procedures.ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

Interview patient about current problems, medications, and allergies and reviews medical record for pertinent previous and current information. Perform in home non-skilled visits to patients and scheduled private duty supervision visits. Coordination with community services, MD offices and appropriate agencies for optimal client care Perform LNA supervision on nonskilled patients. Assist in phlebotomy, identifies patient, checks requisition form for requested tests, and prepares equipment, patient and puncture site. Organize and operates foot clinic under Nurse Practice Act guidelines. Maintain adequate inventory of examination room supplies and prepares exam rooms at the beginning and end of day and cleans between each patient contact.Perform variety of office responsibilities:
answers phones, triages calls, obtain verbal and written orders, faxing and tasks. Facilitate and assists in the coordination of community clinics and community education. Participate in the orientation and training of new personnel. Perform other duties assigned.Qualifications:
E D UCATION AND EXPERIENCE
Graduate of an accredited school of nursing. Current State of New Hampshire or Compact State licensure/registration as an RN. Minimum of 1-year experience in community health or home health setting preferred.SKILLS AND ABILITIES
Ability to work independently and without immediate supervision. Excellent patient care skills. Strong knowledge of nursing and health care terminology. Ability to demonstrate a caring, compassionate and empathic manner with patients and family members. Ability to work as a team and maintain positive working relationships with employees and healthcare professionals. Strong verbal and written communication skills. Ability to document patient care according to nursing standards and plan of care. Must demonstrate ethical conduct and build trust with patients and family members. Ability to follow safety guidelines as instructed by the agency. Ability to wear protective clothing such as mask, glove, head cover, lead apron, safety glasses, safety shoes, etc. Ability to travel required. Travel throughout central New Hampshire is required.
