$40/hr Position Overview We are seeking a dedicated healthcare professional to provide high-quality patient care within a Skilled Nursing Facility (SNF) setting. The ideal candidate will possess strong clinical skills, excellent documentation practices, and a commitment to delivering patient-centered care while collaborating with interdisciplinary healthcare teams. This position offers an opportunity to work in a supportive clinical environment focused on improving patient outcomes and maintaining the highest standards of care. Responsibilities Provide direct patient care in accordance with facility policies, procedures, and standards of practice. Monitor, assess, and document patient conditions and responses to treatment. Administer medications and treatments as prescribed. Maintain accurate, complete, and timely patient records and documentation. Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care. Ensure compliance with infection prevention, patient safety, and regulatory standards. Educate patients and family members regarding treatment plans and care needs. Respond promptly and appropriately to changes in patient conditions. Participate in care planning and interdisciplinary team meetings. Perform additional duties as assigned to support patient care and facility operations. Required Qualifications Licensure & Certifications Active applicable State License required. Current BLS Certification required. Experience Skilled Nursing Facility (SNF) experience preferred. Skills & Competencies Strong clinical assessment and patient care skills. Excellent communication and interpersonal abilities. Strong documentation and organizational skills. Ability to work effectively in a fast-paced healthcare environment. Ability to work independently and collaboratively within a multidisciplinary healthcare team. Strong critical thinking and problem-solving abilities. Additional Information 13-week contract assignment. Skilled Nursing Facility (SNF) setting. Multiple facility opportunities available.
