- Develop individualized care plans in collaboration with the patient's physician, patient, family, and primary caregivers upon admission; periodically review and recertify care plans as appropriate.
- Coordinate patient care through interdisciplinary communication and case conferences; advise other disciplines of discharges in a timely manner.
- Provide ongoing assessment of patients' clinical, functional, and emotional status; communicate significant changes to the physician and care team colleagues.
- Document evaluations, treatment goals, and objectives accurately and in a timely manner.
- Educate patients and caregivers on self-care techniques, medications, diet, and other physician-ordered instructions; utilize opportunities for health counseling. Additional Information Help for Additional Information. Required Qualifications
- Active Registered Nurse licensure in Vermont and New Hampshire, or an unencumbered compact nursing license.
- Graduate of an accredited school of nursing.
- Minimum of 2 years of experience in home health nursing. Preferred Experience / Skills
