  • Home Health Location White River Junction, VT (with coverage across multiple VT and NH areas) Schedule / Job Type Contract | Full-Time | Days | Onsite / Field-Based Key Responsibilities
  • Develop individualized care plans in collaboration with the patient's physician, patient, family, and primary caregivers upon admission; periodically review and recertify care plans as appropriate.
  • Coordinate patient care through interdisciplinary communication and case conferences; advise other disciplines of discharges in a timely manner.
  • Provide ongoing assessment of patients' clinical, functional, and emotional status; communicate significant changes to the physician and care team colleagues.
  • Document evaluations, treatment goals, and objectives accurately and in a timely manner.
  • Educate patients and caregivers on self-care techniques, medications, diet, and other physician-ordered instructions; utilize opportunities for health counseling. Additional Information Help for Additional Information. Required Qualifications
  • Active Registered Nurse licensure in Vermont and New Hampshire, or an unencumbered compact nursing license.
  • Graduate of an accredited school of nursing.
  • Minimum of 2 years of experience in home health nursing. Preferred Experience / Skills
  • Experience with interdisciplinary care team coordination in a home health or community-based setting.
