  • Hospice Location Lebanon, NH Schedule / Job Type Full-Time, Days, Contract, Onsite Key Responsibilities
  • Develop individualized care plans in collaboration with the patient's physician, patient, and family or primary caregivers upon admission; periodically review and recertify care plans as appropriate.
  • Coordinate patient care through interdisciplinary communication and case conferences.
  • Provide ongoing assessment of the patient's clinical, functional, and emotional status; communicate significant changes to the physician and care team colleagues.
  • Document evaluations, patient responses, and progress toward established goals.

  • Active Registered Nurse licensure in Vermont and New Hampshire, or an unencumbered compact nursing license.
