- Develop individualized care plans in collaboration with the patient's physician, patient, and family or primary caregivers upon admission; periodically review and recertify care plans as appropriate.
- Coordinate patient care through interdisciplinary communication and case conferences.
- Provide ongoing assessment of the patient's clinical, functional, and emotional status; communicate significant changes to the physician and care team colleagues.
- Document evaluations, patient responses, and progress toward established goals.
- Educate patients and caregivers on care techniques and end-of-life symptom management. Additional Information Help for Additional Information. Required Qualifications
- Active Registered Nurse licensure in Vermont and New Hampshire, or an unencumbered compact nursing license.
