Mon
- Fri, 12:00pm
- 8:00pm or 11:00am
- 7:00pm.
- Mon to Fri with weekend and holiday rotation.

We are now utilizing EPIC for our electronic medical records!Service Areas:
Patient homes in Southern New Hampshire greater Merrimack, Nashua, Manchester communities. Responsibilities Completes an initial home health assessment of patient and family to determine hospice care needs. Provides a complete physical assessment and history of current and previous illness(es). Utilizes the nursing process in planning, providing and evaluating skilled needs. Completes a comprehensive patient assessment. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and family on the planning and revising process. Qualifications Graduate of an accredited school of nursing. Associate degree in nursing. Registered as RN with New Hampshire's Board of Nursing. A minimum of one year recent acute care experience. Ability to exercise initiative and independent judgment. EPIC experience preferred, willing to train! Preferred Qualifications Bachelor degree in nursing. Community Health/Education experience. Certification in Hospice and Palliative Care, Oncology or end-of-life experience is preferred, but willing to train the right candidate. EMR software proficiency. Required Registered as a RN with New Hampshire's Board of Nursing. CPR Certification. Satisfactory criminal record. Satisfactory motor vehicle record. Once fully trained minimum requirements is one weekend a month pays additional $4.00 an hour and 1 winter and 1 summer agency holidays required pays time and a half.
