  • Nursing license
  • BLS•Current •2 positive clinical references (instructors) •Photo ID |Social Security Card •Physical (annually) •TB Test (annually) •Vaccination|
Titers:
MMR, Varicella, Hep B
  • Flu Vaccine, Tdap or declination
