Position Overview/Purpose:
We are looking for an experienced Per Diem Nurse Practitioner to provide face-to-face assessments care to hospice patients in the Bergen/Hudson County, NJregion. You will apply medical techniques and principles under the supervision of a physician and you will care for a diverse set of patients with chronic diseases. Principal Duties and Responsibilities (Essential Functions): Conduct visits with hospice patients in and near the Bergen/Hudson County, Nj region Work independently under the supervision of a physician Complete physical exam and review of systems and assess eligibility for hospice Speak with patients, families, and caregivers during visits and as needed Attend IDG meetings when possible Prescribe medications supplies and appropriate medically necessary interventions Full-time employees qualify for the following benefits: Medical, Dental, Vision and supplementary benefits such as Life Insurance, Short Term and Long Term Disability, Flexible Spending Accounts for Medical and Dependent Care, Accident, Critical Illness, and Hospital Indemnity.
