PointClickCare LPN Full Job Description Home Health Registered Nurse Shift Requirements :
M-F 8 am-5pm, weekend: On Call Weekly Case Conference meeting on Wednesday at 845am
Benchmark Requirement :
30 points per week M-F or higher depending on patients / agency needs. May be a bit higher. Weekly Case conferences (Wednesdays 8:45am for 1.5 Hrs.) Timely Documentation, within 24 Hours.
License Requirements:
Registered Nurse unencumbered
License Benchmark Requirement :
30 per week Mileage reimbursement : .43 cents per mile Counties covered: Bergen county
NJ Preferred Skills :
Home care home base (HCHB), Point care, Point click care, organizational skills, Technology, Microsoft Teams. May orient novice clinicians if needed. Timely Documentation, within 24 Hours. Assess patients and provide nursing care according to care plans. Monitor health conditions and promote disease prevention. Coordinate care with physicians and other healthcare providers. Manage medications, including ordering, monitoring, documentation, and safety compliance. Report changes in patient condition to physicians. Maintain accurate nursing documentation and health records. Schedule and follow up on medical appointments. Supervise and support LPN/LVN staff as needed. Ensure infection control and safe healthcare practices. Communicate with hospitals during patient admissions and discharges. Train new and existing staff on health, safety, and patient-specific care needs. Provide case management and oversee overall patient health and safety. #
