Job Description

Join our growing healthcare team as a VitalCare Nurse, where your work will directly improve the health and well-being of patients in the community. This role focuses on preventive healthcare, wellness monitoring, and compassionate in-home patient support. As a VitalCare Nurse, you will perform routine wellness visits, monitor vital signs, provide patient education, and help individuals maintain healthier lifestyles through proactive care. This position is ideal for nurses who enjoy flexibility, patient interaction, and community-based healthcare outside of traditional hospital settings. Nurses will travel to patient homes and community locations to provide personalized care and wellness support. Duties Conduct routine wellness visits and patient assessments Monitor vital signs including blood pressure, heart rate, oxygen levels, and glucose readings Document patient information accurately using electronic medical records (EMR/EHR) systems Provide patient education focused on preventive care, healthy lifestyle habits, medication reminders, and chronic condition management Coordinate with healthcare providers and family members when necessary Support elderly patients and individuals requiring ongoing wellness monitoring Maintain professional, compassionate, and patient-centered care during all visits Travel to patient homes and assigned locations as needed Qualifications Active LPN or RN license in the state of employment Strong communication and patient care skills Experience in home care, community health, wellness care, or geriatric care preferred Ability to work independently and manage flexible schedules Basic knowledge of EMR/EHR systems preferred Valid driver's license and reliable transportation required Compassionate, dependable, and professional attitude Benefits Flexible scheduling Competitive hourly pay Opportunity to grow with an innovative healthcare startup Meaningful work that positively impacts community health Supportive and collaborative work environment
