  • Hicare is a rapidly growing healthcare technology company specializing in Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and value-based healthcare solutions.
We partner with physician practices, healthcare organizations, and health plans to improve patient outcomes while helping providers deliver high-quality care beyond traditional office visits. We are seeking an experienced and compassionate Registered Nurse (RN) to join our team.
  • Position Overview
  • The CCM Nurse Care Manager supports Medicare patients with chronic conditions through care coordination, patient education, and chronic disease management. This role also partners with physician practices to support CCM implementation, patient enrollment, and program growth.
  • Key Responsibilities
  • 1.
Patient Communication & Support Conduct phone consultations to assess patient progress and chronic condition concerns. Motivate patients to adhere to care plans and daily vital monitoring. 2. Clinical Data Management Document detailed clinical documentation and Medicare-compliant CCM time logs. Update patients' care plans monthly based on EMR documentation and provider recommendations. Document patient symptoms, medication issues, and lifestyle concerns for provider review. 3. Provider Collaboration & On-site Visits Visit clinics regularly to review patient status, escalate alert cases, and obtain provider directives. Communicate closely with providers to support clinical decision-making and care plan updates. Ensure documentation accurately reflects physician instructions and follow-up orders. 4. Compliance & Documentation Follow Medicare billing requirements for CCM services Maintain HIPAA compliance and protect patient confidentiality at all times.
  • Qualifications
  • Required Active Registered Nurse (RN) License in New Jersey Minimum three (3) years of clinical nursing experienc Strong communication and patient education skills by phone Excellent organizational and documentation abilities Experience using EMR/EHR systems Ability to travel locally to physician offices as needed Preferred Chronic Care Management (CCM) experience Care Management or Case Management experience Medicare population experience Primary Care, Internal Medicine, Family Medicine, Geriatrics, or Home Health experience Bilingual English/Korean Experience working with value-based care programs •Work Schedule •Monday-Friday, 9:00 AM - 6:00 PM (40 hours/week)
Pay:
$45.00 - $52.00 per hour
