Full-time Description:

The team nurse conducts ongoing assessments and educational sessions with consumers in the community and in their residences. The nurse works with consumers to assess needs and identify goals to enhance their independence. The team nurse serves as an educational resource for staff and consumers, provides medication monitoring and counseling as necessary, and ensures linkages to community medical and behavioral healthcare resources.

Responsibilities:

Provides initial and annual nursing assessments to all consumers Completes CRNAs, IRPs, and other CSS Clinical documentation as applicable Works in conjunction with full team to develop individual rehabilitation plans designed to address medical and wellness needs Provides education regarding medical concerns, medication, nutrition, exercise, and wellness to consumers Documents observations, information and services provided to the consumer in progress notes Communicates observations and information about the consumer to the Team Leader and Director of Nursing Provides referrals and linkages to community medical and behavioral health resources Acts as a liaison to community medical providers Ensures all reports and clinical documentation are completed and filed in a timely manner All other duties as assigned or as necessary

Qualifications:

Individual must possess a current and active RN license. Experience with psychiatric conditions and/or behavioral health issues preferred. Valid NJ Driver's License with a clean driving record is required Working knowledge of and ability to work on computers, proficiency in Microsoft Office, and experience working with an EHR is mandatory.
