Compensation:
$53 - $56 hourly
Responsibilities:
Coordinate and manage comprehensive care plans tailored to each patient's unique needs, ensuring a holistic approach to hospice care. Conduct regular home visits to assess patient conditions, providing hands-on nursing care and symptom management. Collaborate with interdisciplinary team members to develop and implement effective care strategies, fostering a supportive environment for patients and families. Educate patients and their families about disease processes, treatment plans, and self-care techniques, empowering them with knowledge and confidence. Maintain accurate and timely documentation of patient care activities, ensuring compliance with regulatory standards and company policies. Serve as a compassionate advocate for patients, addressing their concerns and facilitating access to necessary resources and support services. Participate in ongoing professional development opportunities, enhancing your skills and contributing to the overall excellence of our care team.
Qualifications:
Current Registered Nurse (RN) license in the state of New Jersey. Experience in hospice or palliative care, demonstrating a compassionate approach to patient management. Ability to develop and implement comprehensive care plans tailored to individual patient needs. Proven track record of collaborating effectively with interdisciplinary teams to enhance patient care. Strong communication skills to educate and support patients and families, fostering confidence and understanding. Proficiency in administering medications and treatments, prioritizing patient comfort and safety. Excellent documentation skills to maintain accurate and timely patient records, ensuring clear communication with the care team.
