$2,300 - $2,500 per week Job Summary Pride Health is seeking an experienced Home Care Registered Nurse (RN) for a contract opportunity in Morristown, NJ. The Home Care RN will provide skilled nursing services to patients in their homes, coordinate care with interdisciplinary teams, and ensure the delivery of high-quality, patient-centered care. The ideal candidate will have strong home health experience, OASIS proficiency, and the ability to work independently while managing a diverse patient caseload. Responsibilities Conduct comprehensive assessments of home health patients to identify physical, psychosocial, environmental, and educational needs. Develop, implement, evaluate, and update individualized Plans of Care (POC). Coordinate patient care with physicians, family members, caregivers, and interdisciplinary healthcare teams. Monitor patient progress and modify care plans as needed to ensure optimal outcomes. Maintain accurate and timely clinical documentation, including OASIS assessments and visit notes. Educate patients and families regarding disease management, medications, home safety, nutrition, and self-care practices. Perform skilled nursing procedures including wound care, catheter management, IV therapy, venipuncture, and medication administration. Assess and manage patient pain using appropriate interventions and education. Ensure compliance with infection control standards, agency policies, and regulatory requirements. Supervise and provide clinical guidance to home health aides and other support staff. Facilitate referrals and coordinate community resources when appropriate. Participate in interdisciplinary team meetings and case conferences. Maintain confidentiality and provide compassionate, patient-centered care. Requirements Active New Jersey Registered Nurse (RN) License required. Current CPR certification required. Current driver's license, reliable transportation, and automobile insurance required. Minimum 1-2 years of clinical nursing experience required. Minimum 2 years of Home Health experience required. Homecare Homebase software experience required. OASIS experience required. Strong knowledge of medication administration, wound care, IV therapy, catheter care, and home health nursing practices. Ability to work independently and manage multiple patient visits effectively. Excellent communication, documentation, and organizational skills. Preferred Qualifications Bachelor of Science in Nursing (BSN). CHPN (Certified Hospice and Palliative Nurse) certification. Experience with
EPIC EMR.
Previous hospice or advanced home health experience. Strong care coordination and case management skills.
